We are back! After a two-week hiatus in which we aired the 'Best of' shows while Dr. Gayoso attended the ASAPS Meeting in Vancouver, today's show was up and running with callers very quickly. Dr. Gayoso and Sharon briefly discussed some of the highlights of the ASAPS Annual Meeting, including the exciting news of the FDA approval for the latest form stable silicone implants from Sientra.
Our first call, from Lisa in Pinellas Park, was about eyelid surgery (blepharoplasty) as was our next call from Kay, in Tampa. Both callers have undergone upper or lower blepharoplasty and had some questions regarding additional procedures or revisional procedures. Much of Dr. Gayoso's discussion included the latest regarding microfat injections to restore volume to the face, especially the eye region.
Brad, from Brandon, called regarding scars from extensive back surgery and the options for improving them. Dallas, from Tampa, had an interesting question concerning facial reconstruction, computer programs that are available to assist plastic surgeons in planning complex facial reconstruction and the skillset required. Dr. Gayoso gave a very detailed and honest answer about this subject and paid homage to the pioneers in plastic surgery by saying that modern plastic surgeons stand on the shoulders of their predecessors. Thomas, from Tampa, had questions about tip rhinoplasty revision and finally the discussion turned back towards blepharoplasty and 'new' techniques, such as transconjunctival blepharoplasty. This technique, though not new to plastic surgery, has gained popularity in the news being touted as the latest advancement for eyelid surgery. In the right patient, this technique is quite successful. However, Dr. Gayoso reminded the listeners that there might be some issues regarding eyelid aging that can not be addressed with this technique.

It's all about chin implants!!!!!! This week, on several of the national news shows, Dr. Mehmet Oz discussed 'new advances' in facial rejuvenation with chin augmentation. Of course, those of us in plastic surgery have known how impactful chin augmentation can be on a person's facial structure and balance for many, many years now but it is always nice when the media catch on to 'our little secrets'. Today's show focused considerably on chin augmentation (chin implant surgery). Dr. Gayoso discussed the two surgical methods of making a chin more prominent either by moving the jaw bone forward (genioplasty) vs placing a solid silicone implant in the chin ( chin augmentation). Genioplasty is not as commonly done since there are so many types and styles of chin implants. Augmenting the chin can be a very nice way of helping the jawline to appear more slender and balancing out the facial structures. The nose/mouth/chin structures have alot of influence on each other with regard to facial aesthetics and it is important to take all three structures into consideration when making changes to the face.
We did have some non-chin implant discussion prompted by a few callers. David, from St Petersburg, had questions regarding reducing the mid-section in men. This can be a difficult area to treat with liposuction due to the fact that the fat sometimes can be intraabdominal. Often times, weight loss is the best 'treatment' for this problem area.
Breast lift and nipple preservation was discussed. During mastopexy surgery, the nipple is not removed (contrary to some reports). With breast reduction surgery, in cases where the breasts are very large, nipple reconstruction w/ free nipple graft may be required. This occurs only in a small percentage of breast reduction cases.

Wow! The radio listeners have definately found us. Lots and lots of calls on today's show. Our first caller, Dawn, had a question about tummy tuck surgery and the various methods of treating fat removal and excess skin, including cold therapy (Cool Sculpting, Zeltiq), Thermage, radio frequency and several others. Dr.Gayoso discussed each application,including the fact that some of the procedures are very new and have not yet proven long term efficacy (Cool-Sculpting/Zeltiq). We will continue to follow each of these therapies closely! Surgical treatment for excessive abdominal skin (tummy tuck) is a proven procedure and remains the gold standard, though it is invasive and requires a few weeks of recovery.
Sharon and Dr. Gayoso discussed lip augmentation and the importance of not overdoing it and maintaining a natural appearance, despite the Hollywood trend for aggressive facial augmentation.
Mike, from Riverview, called with questions regarding breast implants and mammograms. The discussion included the importance of having mammograms performed by facilities that are familiar with the extra views required for patients with implants.
Kathy, from Venice, had questions on treatment of capillaries on heh face and age appropriateness of Botox. Facial veins can be treated with laser therapy with nice success, though more than 1 treatment may be required. There is no age limitation for Botox. As always, healthy patients with no other contraindication for Botox are the patients who are appropriate candidates for Botox.
We heard from a Hudson viewer, Vivian, regarding miracle creams for treatment of facial wrinkles....and unfortunately, despite aggressive marketing, there is no wrinkle cream that can reliably eradicate facial wrinkles. We do know that many of the prescription skin care lines, such at Obagi, do have a nice effect on the overall skin appearance by evening out skin tone and texture.
Lynn, from St. Petersburg, had a facelift 1 year ago by another physician and had questions about persistent redundant skin behind the ear. This can be treated with a minor in office procedure!
David called with a question about post basal cell carcinoma scar on the nose. These deformities can often be made to look a bit better but surgical intervention is required in the form of flap surgery.
Finally, Katherine from Odessa called asking about the difference between Aqua-lipo, laser lipo, Vaser lipo etc. All of these procedures involve breaking up the fat and removal with liposuction. The type of machine may vary but the overall improvement, if done by a skilled plastic surgeon, can yield nice results. Unfortunately, these machines are marketed to non-plastic surgeons as well, and sometimes the results are not as predictable. We always encourage our listeners to choose highly trained, board certified plastic surgeons for their plastic surgery needs!

There was 'no foolin' on today's show.....a wonderful discussion about the newsworthy topic of facial transplantation and the strides that have been made with this technique opened the show....face transplantation remains a topic that fascinates many and will always be a great subject to discuss.
John, from St. Petersburg, called with questions and concerns about post-facelift redness. Some of the ways to treat this involve pulsed dye laser to help diminish the redness in the cheek regions.
Dee, from Pinellas, called with an update to her call from several weeks ago involving her post lower lid blepharoplasty issue of ectropion. The treatment options, including time, were discussed with her and she will give us periodic updates on how she is healing.
Mary, from Largo, asked to have clarification of traditional liposuction (suction assisted lipectomy) downtime, volume limitations and long-term results, including fat redistribution. Dr. Gayoso was able to provide her with information about traditional liposuction and reinforced that patients who are at their stable weight will provide the most predictable and long lasting results.
Michelle, of St Petersburg, called with questions about the highly marketed "lifestyle lift". We receive calls nearly every show about this largely marketed procedure and try to help the listener realize that the results on the print ads and the commercials show patients who have had 'optional neck firming or eyelid firming' procedures in addition to their 'lifestyle lift'. Dr. Gayoso encourages all patients to spend time discussing the procedure that is best for them with their potential surgeon.
Fred, of St. Petersburg, called with concerns about seeing people who appear to have had too much plastic surgery. Dr. Gayoso agreed with Fred that looking as if you have had plastic surgery is very bad for business and our goal at Gayoso Plastic Surgery is to have patients look natural and not appear to have had surgery. We want patients to look good but not look as if they have been 'operated on'.

Today's show touched on the subject of illegal and inappropriate cosmetic surgery performed by non-plastic surgeons, such as beauticians, with sometimes life-threatening results. Unfortunately, the headlines offer too many examples of patients seeking cosmetic surgery thru avenues that don't involve board certified plastic surgeons. One of the goals of this radio show remains education of the listening public so that if a person decides to embark on having a cosmetic procedure done, we have empowered them on how best to research the credentials of their potential "surgeon".
A new subject that has not been discussed on former shows of ours was brought up: labiaplasty (female genitalia rejuvenation). Once thought of as a taboo subject, more and more women are realizing that issues of the female genitalia can be addressed surgically (with good success) by plastic surgeons.
Several callers phoned in asking questions about reconstructive and cosmetic procedures, including scar revision from congenital surgery to correct esophageal atresia, skin tightening with one of the various highly marketed machines promising results that may not be as dramatic as the advertising leads us to believe, and revisional breast implant surgery after heart valve replacement.
Dr. Gayoso spent some time discussing the future breast implant on the horizon: form stable implants and how they differ from the current cohesive gel (gummy bear) implants.

Spring break is upon us and the radio show questions about arm surgery (brachioplasty) prove that this issue is a common complaint among women. Laxity or looseness of the upper arm skin can be very bothersome and results in many women wearing longer sleeves, even in our beautiful weather, in order to hide unsightly upper arm skin looseness. Dr. Gayoso and Sharon spoke about this issue and how it can be surgically corrected with arm surgery. The limitations of this procedure include scars that are permanent and can sometimes be unsightly. Persons undergoing brachioplasty surgery will trade shape with scars. The scars are placed as inconspicuous as possible but are still there and remain one of the biggest hurdles for arm lift surgery.
Our first caller had additional questions about brachioplasty as well as questions about facial rejuvenation with Botox used in the glabella (frown) lines.
A shy caller inquired about thigh liposuction to correct fullness of thighs in a person who has otherwise normal body shape and size. Liposuction is ideal in situations where the patient has localized adiposity (fat) that does not respond to diet or exercise.
Kathy, from St. Petersburg, called with questions about upper and lower eyelid surgery (blepharoplasty surgery). Dr. Gayoso provided a nicely detailed discussion about how the upper eyelid and brow can work in tandem to cause upper eyelid aging. Jane, from Palm Harbor, also had questions about eyelid surgery, specifically which types of doctors are qualified to perform eyelid surgery. As always, Dr. Gayoso educated her on choosing a board certified plastic surgeon who is experienced in eyelid surgery. He did, however, agree that opthalmologists who have done advanced training (fellowship) in eyelid surgery are also qualified to perform blepharoplasty surgery and he acknowledged that he has several opthalmology trained peers whom he feels are very qualified and whom he has a great deal of respect for. As always, the bottom line is that we want patients to research the education and training of the surgeon and look for board certification in plastic surgery. Board certification in Internal Medicine, Family Practice (or any of the other specialties that are branching into cosmetic surgery ) does not qualify one to perform cosmetic surgery. It takes more than a weekend course to become proficient and skilled in plastic surgery.

Another interesting and varied show with some good discussion about elective cosmetic surgery and the social implication of it. More on that in a moment.
The show started our with an early phone call from a patient asking about fat cell storage in anticipation of a future use. Though it sounds intriguing, science has not yet perfected allowing storage of our fat cryogenically to allow for use of that fat in the future! I am sure one day that will happen, but we are just not there yet. It did offer Dr. Gayoso the chance to remind all of us that plastic surgeons, by the very nature of their specialty, often have to be quite creative in order to meet the reconstructive needs of patients whom they deal with on a daily basis. That means there's never a dull moment in the day of a plastic surgeon.
The news of the week provided some discussion on a pending legistative decision regarding office based liposuction procedures being performed under local anesthesia and the inherent dangers that this might pose with large, sometimes toxic levels of local anesthesia required in order to keep a patient comfortable. Dr. Gayoso reminded all of us that safety is foremost when we are contemplating cosmetic surgery and the marketing efforts touting liposuction with 'no anesthesia, no downtime and no pain' are too good to be true.
Dr. Gayoso briefly discussed the recent FDA approval of Sientra silicone breast implants. I am sure we will have more information on this in the very near future.
We received a call from a physician who felt like plastic surgery was much too common and that patients were jumping into a procedure without realizing it was real surgery with the potential for real long term issues. Dr. Gayoso agreed that cosmetic surgery is an elective procedure and that it is very important to our office to make sure patients are well selected to undergo the procedure and that patients are well educated and informed about the procedure. We hope that the listeners of our radio show realize that Dr. Gayoso and the staff at Gayoso Plastic Surgery place a significant amount of emphasis on patient education and feel it is our duty to make sure patients understand that cosmetic surgery is 'real surgery' and not something to be taken lightly.

A show packed with various questions and information about cosmetic surgery, reconstructive surgery and even some dermatology. Our listening audience is spending the first part of the show listening to Dr. Gayoso and Sharon talk about leading headlines pertaining to plastic surgery. About mid-way thru the hour, callers jump in and call with their questions. This week, we had questions about skin care and sun exposure, scar revisions, and scalp reconstruction. On the cosmetic side of surgery, we had 2 callers inquire about eyelid surgery, (blepharoplasty) and how to correct a complication that can occur, though rarely, after lower eyelid surgery (ectropion). The very next caller then inquired about upper eyelid surgery and entropion. Both of these callers had extremely relevant questions about eyelid surgery and possible complications.

Today's show, though abbreviated due to the hockey game, was highly successful once again. Thanks to the wonderful article in the Tampa Bay Times about Dr. Gayoso and his show, we had an abundance of callers this week. These calls included calls from a few former patients of Dr. Gayoso's, including a patient who has completed the newest form of breast reconstruction using fat grafting. Our patient was so gracious to call in and give her personalized story of her journey through fat grafting in order to rebuild her breast. She had a mastectomy nearly 20 years earlier and was finally tired of having to wear an external prosthesis. Her words of experience and wisdom with this procedure will certainly be helpful for the many women who are in a similiar situation.
Other topics included use of collagen (Acellular dermal matrix, as it is known in the industry) for breast reconstruction when we use breast implants or tissue expanders, male facelift surgery and ways to have it look natural, mini-tummy tuck surgery and abdominal liposuction from a patient who had triplets several years ago and selected a surgeon who may not have been trained in plastic surgery. Ways to correct some of the abdominal irregularites that resulted from this procedure were discussed with this patient.
This week's show had another caller inquire about the highly marketed procedures for facial rejuvenation (Lifestyle Lift, Image Lift etc) and once again, Dr. Gayoso educated the patient on facial rejuvenation and the importance of choosing a board certified plastic surgeon who will taylor the procedure to the individual's needs rather than perform a cookie cutter type procedure that is easy to market but may not be for everyone.
A thank-you is in order to Richard Martin and the Tampa Bay Times newspaper for writing a wonderful article highlighting Dr. Gayoso and the show. This week's show was packed full of several different plastic surgery topics. Dr. Gayoso gave a brief history on the origins of Plastic Surgery, dating back nearly 2500, including early developments of flap reconstruction, particularly in the facial region. We had several callers asking questions of Dr. Gayoso. These included a question from a female caller inquiring if the type of liposuction had any influence on a patient developing a blood clot in the leg. The same caller had questions about whether she was a candidate for a necklift or not. This led into further discussion on all of the popular marketing monikers for facelift surgery. When it all boils down to it, they all represent some form of facelift surgery. Another listener called in with further questions regarding these TV facelift ads as well.
We received a call regarding orthognathic jaw surgery and the need for a chin implant. This patient had her jaw moved many years ago and felt that she still needed more prominence of her chin region. This can be accomplished very easily with a chin implant.
Medical tourism and the risks vs. the benefits were discussed in detail. Dr. Gayoso reminded patients that although it is tempting to travel to worldly places for less costly cosmetic surgery, follow-up care can be difficult to obtain and regulations of devices and procedures may not be as thorough as we are used to in the US. Again, discussion of the current PIP implant scandal that is plaguing other countries was reviewed.
Overall, it was a wonderfully interactive radio show and we are very greatful for all of the positive feedback. Our next show, 2-26-12, falls on a day that the Tampa Bay Lightening are playing so our show will be aired for 30 min only and then recorded for the final 30 minutes. Another wonderfully informative radio show! Topics included everything from State Fair Food and how good AND bad it is for you to general body shapes and sizes. Dr. Gayoso answered several calls, including a call from a slender mid 40 year old female who desired a mini-tummy tuck. She wondered how he determines if a patient is a candidate and had several questions specific to her particular problem. We are hearing alot from our male listeners: this week it was about tummy tuck surgery in the male population. Dr. Gayoso explained how men tended to store body fat around the viscera (organs) as opposed to storing fat in the skin and fatty layer. This may mean that a tummy tuck might not get the male patient the results that he desired. Men tend to do very well with liposuction of the love handle region however and a discussion about this procedure ensued. Be sure to catch the podcast of this show on the WFLA website! Dr. Gayoso discussed an upcoming expose on a local TV station exposing the State Attorney General investigation of deceptive marketing practices of the nationally marketing “LifeStyle Lift” cosmetic surgery business. The investigation was highlighted in USA Today in September 2011 and has specific ties to Florida.
More discussion on breast cancer reconstruction, specifically in a patient on whom radiation was performed was also discussed.
Finally, a patient posed a question about when can a mini facelift vs a full facelift be performed and if there is an age limit on who can undergo facial surgery.
